Revision Parathyroid Surgery – Challenges and Considerations in Comparison to Primary Surgery

Abstract Aim Revision parathyroidectomy is made necessary by recurrent and persistent parathyroid disease. The aim of this study was to identify specific challenges in revision surgery compared primary surgery. Methods All parathyroidectomies performed a single surgeon tertiary referral centre over 17- year period were identified. Cases assessed for: demographics; imaging; histology; biochemistry; gland weight, location, ectopia; cure rate. Results consecutive series 100 parathyroidectomies. Statistical analysis using SPSS software. 28 surgeries identified (18 persistent, 7 recurrent, 3 insufficient data). Demographics well matched. Ultrasound correctly localised the pathological 18% cases, 37% cases. Technetium-sestamibi scanning 71% 69% Preoperative calcium significantly higher cases (2.88 vs 2.80, P<0.05). There no significant difference post-operative calcium, pre- or hormone levels, location. glands excised ectopic, 24% (P=0.16).
